Reconstructing the Reconstruction

In the past few years, we have seen a quantum shift in the treatment of breast cancer.  Medical treatments for breast cancer – chemotherapy, hormone treatments, and genetic treatments – have changed significantly, and although no cancer treatment is “easy,” newer therapies are much more effective and patient-friendly than in the past.  Likewise, radiation treatments have become more targeted, meaning less “normal” tissue is affected.

Surgical treatments for breast cancer have also undergone significant changes.  Here are just a few:

» Mastectomy techniques have changed for the better.  Gone is the “breastbone to armpit” scar of the “old” mastectomy; a modern mastectomy respects the anatomy and shape of the breast instead of destroying it.  Contemporary mastectomy incisions are designed like breast reduction or breast lift incisions with the assistance of the plastic surgeon responsible for reconstruction.

» Nipple-sparing mastectomy techniques have been proven safe and effective.  In the right patients, a mastectomy that spares the nipple can result in a reconstruction that is difficult to differentiate from a normal breast.

» The “lumpectomy,” or partial mastectomy, is becoming more common.  In women who are able, removal of the tumor itself, sparing the rest of the breast, is becoming a more common form of surgical treatment.  The lumpectomy must be combined with radiation treatment but with newer forms of radiation (such as implantable radiation treatments or “partial breast” irradiation).  This is quickly gaining favor among breast cancer patients.

» Lymph node removal is rarely performed anymore.  With the advent of targeted “sentinel” lymph node biopsies, removal of the lymph nodes of the armpit are no longer routinely performed.  Removing the lymph nodes can cause severe arm swelling, which is difficult to treat without sophisticated surgery.

» Breast reconstruction is being planned as part of the cancer treatment process.  The most important aspect of a cosmetic breast reconstruction is that as much normal tissue (skin and the fat immediately under the skin) is left behind as possible.  Collaboration between the general surgeon and plastic surgeon BEFORE surgery is essential for this to occur.   

The benefit of breast reconstruction for a woman’s psychological and physical well-being has been well documented.  Proper tumor removal is the most important factor in a successful reconstruction, and “newer” techniques that respect the anatomy of the breast are the best first steps to achieving a satisfying reconstructive result.  Once we, as plastic surgeons, are left with a healthy skin “envelope,” any number of techniques can be used to recreate the breast itself.   A few of the newer techniques include shaped silicone implants, fat grafting, and perforator flaps.

Shaped silicone implants.  Newer, breast-shaped implants allow us to recreate the normal shape of the breast and avoid the “stuck-on” look that was common in older reconstructions.  When combined with nipple-sparing mastectomy techniques, these newer implants can allow us to reconstruct a breast in a single stage instead of the three stages that were common in the past.

Fat grafting, where fat is harvested from one area of the body with liposuction and re-injected somewhere else, has allowed plastic surgeons to further recreate the natural shapes and curves of a beautiful breast.

Perforator flap techniques, such as the DIEP flap, allow for reconstruction of the breast without the need for implants.  A DIEP flap is a lengthy, complex operation that requires a few days in the hospital to recover.  Every year more plastic surgeons are adding it to the list of techniques they use for breast reconstruction.

If you are a woman (or know a woman) who had breast reconstruction already, you may have a reconstruction that isn’t satisfying or that you feel you have to “live with” for the rest of your life. This is not the case.  Fortunately, asymmetric, unnatural breast reconstructions can be salvaged or, in some instances, completely transformed using newer reconstructive techniques.

» Hollowness in the upper part of the breast can be eliminated.  A hallmark of older breast reconstruction techniques, this can be eliminated using fat grafting, shaped implants, or a combination of both.

» Breast crease asymmetries can be fixed.  Using newer, shaped implants or acellular dermal matrix, the crease below the breast can be recreated in an even position.

» Old transverse scars can be corrected, either surgically or with the use of laser treatments or fat grafting (which, in addition to adding shape, can significantly improve the quality and texture of overlying skin).

Do You Wish Your Vision Was Perfect?

Many refractive conditions exist, but the main three are myopia (near-sightedness), hyperopia (far-sightedness), and astigmatism (distortion caused by a misshaped cornea or crystalline lens).  Depending on the severity of your refractive error, you may have several options, or you may be limited.

Wearing eyeglasses is the standard method of correcting refractive error.  Since our vision is very subjective, it is important to obtain an accurate prescription from an experienced eye doctor.  Of equal importance is the optician fitting your glasses and the quality of the eyeglasses.  Beware of cheap alternatives to high-quality eyeglasses.

 As the father of four young children, Dr. Andy Turner understands the importance of a beautiful, balanced, and healthy smile.

 EK: Dr. Turner, many people probably share a rather generic view of the field of orthodontics, yet there have been major advancements in this area that benefit your patients.  Describe some of these advancements for our readers.

Dr. Turner: That’s very true. Most people, especially adults, think of orthodontics as the bulky metal braces of the past. But the orthodontic industry has definitely come a long way since our “metal mouth” time period. The braces we use today are much smaller and much more comfortable for our patients. We use special self-ligating Damon® brackets that move teeth even faster and require fewer adjustments than your traditional brackets. They also come in clear, which is a popular option among our teen and adult patients.  In addition to the self-ligating brackets, we offer a completely “wireless” option called Invisalign®, which uses clear, removable aligners to straighten your teeth over time. As you can imagine, this is a top choice for adults.

 EK: The father of four children, how has the patience and understanding you’ve learned at home benefited you with your younger patients and their parents?

 Dr. Turner: Both at home and in the practice, we have a lot of fun. I can easily relate to my patients and their parents.  I understand the struggles that parents have of getting their kids to brush properly, so it’s important that we work as a team.  At the end of the day, I know as well as they do that kids will be kids. But as a team, we can work together to make sure those smiles are cared for in the best way possible.

 EK: I think while many of our readers associate orthodontics with younger people, the truth is that a growing number of adults are seeking your professional services.  Explain.

 Dr. Turner: Yes, that is very true.  More adults today are choosing to get braces.

The only regret they have is that they didn’t do it sooner.  Braces today aren’t what they used to be.  Treatment length is shorter, and if you opt for Invisalign®, no one will even know you are wearing them.  Adults also benefit from improved self-confidence, which can lead to improvements in their professional and personal lives.  

 EK: How important is the communication that both you and your associates have with all of your patients and the parents of your younger patients?

 Dr. Turner: As I mentioned, it’s important that we tackle treatment as a team.  I want my patients and parents to understand what’s happening every step along the way and encourage them to ask questions.  If my patients understand what we are doing and why, I find they are more compliant. And it definitely puts parents at ease having that open communication.

 EK: You were recently honored by Invisalign®  as Knoxville’s Preferred Provider.  Tell us first about Invisalign® and this honor.

 Dr. Turner: Invisalign® is a way to straighten your teeth without braces.  Instead, we use clear aligners that are essentially invisible and can be removed.  The aligners are custom made for each patient, and as you work through a series of aligners, your teeth visibly improve as you progress through treatment.  This option is better suited for some people’s lifestyle and can simplify treatment. We have successfully treated a number of patients throughout the years with Invisalign® and received the designation by Invisalign® as a Preferred Provider.

EK: What words of encouragement can you offer parents whose children might require your services, or adults for that matter?

Dr. Turner: Getting started with braces is extremely easy, and it’s not a decision you will regret. I have seen first-hand how braces can change a person.  It’s amazing watching timid kids covering their mouths when they laugh transform into confident, outgoing teens during just a short period of time. It makes what we do that much more rewarding.  My advice to any parent is to come in for a free evaluation. Knoxville Plastic & Craniofacial Surgery

The Princesses and The Plastic Surgeon

Recently a colleague of mine told me that he does not want his daughter growing up with the label of “princess” because he thinks it causes girls to develop a sense of entitlement and an obsession with the superficial.  I disagreed.  I view it as a term of endearment and told him that not only am I raising princesses, but I’m raising princesses who can change a tire, shoot a gun, and bait a fish hook.

My response got a laugh, but also got me thinking: How do I raise two girls to have a healthy body image and be happy with who they are when I’m a Plastic Surgeon? 

Regardless of what I do day in and day out, Dr. 90210, a character created and perpetuated by the media, is the stereotype of the plastic surgeon. Unfortunately, my daughters will have to contend with that public perception of what their father does as they grow up and begin to define who they are.

Is it hypocritical for me to tell my girls to be happy in their own skin and ignore the messages about being “skinny” and “sexy” that they are bombarded with when I am in the beauty business? How do I teach them to be self-assured and confident in themselves? How do I teach them to be unselfish young women?

After some thought, these are the answers I came up with:
» The girls need to know they’re loved unconditionally.
» They need to develop a healthy lifestyle early.
» They need to develop a generous heart.
» They need to learn the value of hard work and a dollar.
» In trying to teach these lessons, my actions need to speak louder than my words.

Knowing they’re loved
The girls need to know that I love them.  I’m the first man in their life, and I will always hold a special place in their hearts.  I am the barometer against which every other male that comes into their lives will be measured.  They need to know that my love does not come with a fee. They need to know that I love them for who they are on the inside, and nothing can change that.  The only way they will know that is by experiencing that – at home, every day.

Developing a healthy lifestyle
Our girls need to see that living a healthy lifestyle is more important than a dress size. So much of the media’s portrayal of exercise and health is one of deprivation and starvation – frankly, none of it sounds like fun.  My wife and I strive to live a healthy life and to make sure the girls are included.  We plan meals together, we eat together most nights, and we play together.  Outside playtime is important, and we make time for that – we try and make running around and sweating FUN.

Limiting “screen time” is important to us in that it both forces us to DO something and limits the reach of so many of the damaging media messages.  Those things, I hope, will pay off in that we will have healthy, active girls who don’t see eating well and daily exercise as a chore but as a normal way of life.

Growing up with a generous heart
We do not want our girls to be self-absorbed children, so we try our best to NOT model being self-absorbed parents.  We try to model living a life of service and hope that by living the message we’re teaching, they will live it as well.  We hope that they will learn that the needs of others come before their own and that they will understand that a life of service isn’t about their job but is about who they are as a person.

Learning the value of work
Starting now, while they’re young, we do our best to make sure our children know that the material things they enjoy have come at the expense of hard work and sacrifice.  For our oldest, we’ve started a chore chart, and the money she earns every week goes equally into three separate boxes marked spend, save, and give.  We hope this will not only teach her the value of a dollar, but the importance of saving and giving.

A word about cosmetic surgery
I have no doubt that the cosmetic surgery discussion will happen in my house at some point.  I have insights into the topic that few other fathers have and want to share those with anyone considering cosmetic surgery, my own daughters included.  However, what they need to hear is the same message anyone considering cosmetic surgery needs to hear: Cosmetic surgery should not be an outward attempt to create inner beauty; surgery should only serve to enhance it.

I don’t profess to have all the answers to my own questions – my wife and I are just doing all that we can to raise healthy girls with a strong sense of themselves and to equip them as best we can to deal with the pressures of the outside world. All that being said, they’ll always be my princesses.
